All repeat prescription requests must be submitted 48 hours in advance.
Repeat prescriptions may be requested in writing or electronically. We are unable to process repeat prescriptions by telephone.

Repeat prescription forms may be completed and submitted online below.
Patients requiring repeat prescriptions are asked to attend every six months for review with their doctor. This is based on best practice guidelines.
Westmount Clinic operates a policy of accepting repeat prescription request forms by written (including electronic) request only. This policy is in place to:
- Ensure maximum patient safety.
- Ensure that patients are aware of the medications they are requesting.
- Reduce the risk of prescribing unnecessary medications.
- Ensure accurate records of patients medication requests are retained.
