Please arrive at your scheduled time and check in at main reception.
Face masks are no longer compulsory but we kindly request if you have any respiratory/flu like symptoms to please wear a mask while attending the clinic.
Prescription Requests
Please order your prescriptions online or submit a written request to our prescription box available at the front door. Nominate your pharmacy so your prescription can be sent electronically. Please allow 48 hours for completion.
Social Welfare Certification
Social Welfare Certificates are now submitted electronically.
Ongoing Certs will be completed online on the day they are due.
Forms
All paperwork to be completed by a doctor can be dropped into reception. Please allow at least 72 hours for completion.
